How to register as a new patient

Welcome to Alresford Surgery. We are currently taking on new patients.

The quickest & easiest way to register is to do so using this link;https://gp-registration.nhs.uk/J82124

Alternatively you can download and print both PDFs below (the registration form and either the adult or child questionnaire). Fill them in, sign them and bring them into your chosen surgery.

If you are in possession of your child’s immunisation record please bring this with you when you register.
