Forms and Resources
Set out below are links to forms and other downloadable resources for our patients
Home blood pressure monitoring
If your doctor has asked you to record your blood pressure readings, please download the spreadsheet to the right of this page.
The spreadsheet automatically calculates your average readings which will help your doctor to decide on your future treatment.
Please email the spreadsheet back to us at admin.alresford@nhs.net when completed and be sure to include your name, address and date of birth on the form.
TREND Diabetics
If you are taking one of the following medications then this information is very important:
Dapagliflozin, Empaglifozin or Canagliflozin.
These tablets belong to a group of medicines called SGLT2’s . They can increase your risk of a rare side effect known as diabetic ketoacidosis.
Please look at the following links for information and care if you are unwell.

It is important to note that if you are very unwell you should STOP your SGLT2 (flozin) medication and metformin. Never stop insulin
We will also be prescribing some urine sticks that can be used to check for ketones if you are unwell. Please collect these from your usual pharmacy/dispensary.
If you have any further queries, please do not hesitate to contact a member of the diabetic team.
Ear Care
Ear wax is normal and is produced to form a protective coating over the skin in the ear canal. Ears are normally self-cleaning – the movement of your jaw whilst eating and talking helps to move the wax along the canal where it will usually fall out naturally without you noticing.
If there is a build up of wax in your ear(s) please read the self-help guide below;
Coil fitting information
There are two mains types of coil which we routinely insert at the Alresford Surgery.
The Copper Coil (Intrauterine Device = IUD) and the Mirena Coil (Intrauterine System = IUS). Both are extremely effective contraceptive options. There is useful information about the coils and other contraceptive options at www.fpa.org.uk.
Please find below our coil fitting information sheet.
Low Carb Diet Sheet
You can access a low carb diet sheet here, which is particularly useful for our Diabetic patients.
