Alresford Surgery Patient Forum – Survey

Our patient participation group, known as Alresford Surgery Patient Forum has been in existence since 2010.  We are keen to develop this group and the value it provides to the surgery in terms of improving the quality of the services we provide.

With this in mind I would be really grateful if anyone registered as a patient at Alresford Surgery could complete this short questionnaire – it really should only take a couple of minutes.
