ADHD Referral Process

Thank you for your interest in a referral for an ADHD assessment. Please read the following three step process carefully.

STEP ONE:

NHS or Right to Choose Pathway

Due to high demand, NHS ADHD assessment waiting times are currently very long. We are fortunate to still be able to refer on the local NHS pathway but you also have the option to be referred through the Right to Choose pathway. This allows you to choose a private provider, funded by the NHS.
Important: All Right to Choose providers are classed as private providers.
If you wish to use this route instead of our local Hampshire IOW NHS pathway, you must choose a provider yourself. A list of eligible providers can be found on the ADHD UK: Right to Choose website.
Please note: We are unable to select a provider for you.
Right to Choose providers and details can be found here:

ADHD UK: Right to Choose

PRIVATE PROVIDERS

NHS GPs have no obligation to perform any investigations or prescribe any medications following attendance at a private provider. For more information on this in general, there is an excellent explanation on Ivy Grove’s website which can be found here: Private providers

It is vital that, if selecting a Right to Choose provider, you ensure that:
• 1. You meet their referral criteria
• 2. They can prescribe medication for you (if this is something you would wish to pursue)
• 3. You are aware of their waiting times

We appreciate that you will be keen to be assessed as soon as possible, however, please do not choose a provider simply because it has the shortest waiting time. Do check that it is the right service for you.

STEP TWO:

To proceed with a referral

To request a referral for ADHD assessment, you do not need to speak with a GP first. Instead, to streamline the process, we would request that you submit all of the following documents to the surgery either via email to: admin.alresford@nhs.net or in hard copy to reception. Once we receive your full submission, your documents will be reviewed and we will action a referral if appropriate.

DOCUMENTS NEEDED:
1. Completed Hampshire Isle Of Wight possible ADHD referral form which includes the screening questionnaire available below;

Possible ADHD Referral form

Please ensure that you try to include as much information as possible – it can be typed or handwritten, detailing your symptoms, concerns, and how they affect your daily life (home, work, relationships). Also include:
o Strategies you’ve tried
o What you hope to gain from diagnosis
Please be thorough – incomplete submissions may be rejected.
2. Details of your selected referral pathway – either Hampshire IOW NHS referral or a Right to Choose provider
3. If Right to Choose then any completed referral form(s) from your chosen provider’s website
Your GP cannot complete this for you. Please again make sure that any form is fully completed by you before sending this on. Incomplete forms may be rejected.

We can only process your referral once all the above documents are received.
If any documentation is missing, we will send you a reminder. You will have 21 days to respond. If we receive no reply, we will assume you no longer wish to proceed, and your request will be closed. This ensures our team can continue to assist other patients promptly.

STEP THREE:

Referral Review and Next Steps

Once we receive your full submission, the referral will be reviewed and processed..

If you meet the criteria, we will send your referral to your chosen provider and confirm this in a text to you. The provider should then contact you directly.
If you do not meet the criteria, you will be informed and offered guidance on alternative support options.
After referral submission, your GP surgery will no longer be involved in the process. Please direct all future queries to your chosen provider.
